Avenue 360 to host Voices for Hope gala on Nov. 13 | Community Impact

Avenue 360 Health and Wellness believes that a stronger Houston begins with healthier people, stronger families and communities where everyone has access to the care they need to thrive.

For decades, Avenue 360 has been a trusted provider of community healthcare in Greater Houston. Today, the six clinics and Omega House Hospice serve over 13,000 patients a year with whole-person care that is affordable, accessible and centered on dignity.

Avenue 360 believes in 360 degrees of high-quality care with each patient at the center. This support helps sustain primary care, pediatrics, dental care, behavioral health, HIV prevention and treatment, pharmacy services and wellness programs. It also funds the care that happens beyond the exam room: case management, supportive services and housing support, including help for neighbors experiencing unstable housing. When a patient needs more than a prescription, the team helps them to connect the dots.
